If the information reported on IRS Form 1042 is no longer correct the QI must correct this information by filing an amended Form 1042 with the IRS.

Follow the instructions to Form 1042 for amending the form. Make sure to check the box on the very first line to indicate an amended return and, if required by the instructions, include a letter explaining the reasons for amending the form.
